How do I declare my account in the 2021 tax form?
The instructions below are for the 2021 tax form, which you fill in in 2022.
You only need to declare your account if you've invested money in 2021. So you're exempt if you've opened up an account in 2021 but haven't invested anything yet that year.
Add the account to "Vak XIII" under "A. Rekeningen in het buitenland":
Check the box 1075
Fill in your name
Indicate "NEDERLAND" as the country
Check the box "Zijn de wettelijk bepaalde gegevens over de rekening bij het Centraal Aanspreekpunt bij de Nationale Bank van België gemeld?".
Before you check the box "Zijn de wettelijk bepaalde gegevens over de rekening bij het Centraal Aanspreekpunt bij de Nationale Bank van België gemeld?", make sure you declared your account to the Belgian National Bank. See the instructions.
Add the account to "Cadre XIII" under "A. Comptes à l'étranger":
Check the box 1075
Fill in your name
Indicate "PAYS-BAS" as the country
Check the box "Les données relatives au compte, qui sont prévues par la loi, ont-elles été communiquées au Point de Contact Central de la Banque nationale de Belgique ?"
